Based on interview and record review, Area Manager informed LPAs that on 12/29/25 an inappropriate incident happened between two children. 8 by 1/8/26 before the close of business day (5:00 PM). This written plan of correction will detail how such occurences will be avoided moving forward. Pictures of returned LIC 9224's should also be sent to LPAs before the end of the week (1/9/26).
Based on record review the licensee did not comply with the section cited above, Staff 1's mandated reported trainig had expired which poses/posed a potential health, safety or personal rights risk to persons in care.
