Based on record review, the licensee did not comply with the section cited above. S3 did not have current mandtaed reporter training certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.

By POC date: Program Director shalll submit a detailed plan to ensure that children are supervised at all times. Since this is a zero-tolerance violation, an immediate civil penalty of $500 was assessed during the inspection. LPA's observation, interviews conducted, and record review during the investigation it was determined that the Teacher had left 6 preschoolers unsupervided in the classroom when she ran after a child who ran out of the classroom. This poses an immediate risk to the health and safety of the children in care. 8 Upon receipt, licensee shall post and provide copies of this licensing report,and have LIC 9224 signed and kept on file, to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months.
