Site Director stated that she will submit a written plan to Licensing by 07/03/25 to ensure that children's personal rights are not violated. Site Director stated that an All Staff Meeting will be conducted on 07/11/25 and children's personal rights will be included. Meeting agenda and attendance sheet to be sent to Licensing by 07/14/25. On 06/27/25, Staff (S1) violated children's personal rights by grabbing a child (C1) on the waist and sat the child down on the chair, pinching a child (C2) on the arm using a pair of tongs, and withholding food to a child (C1). This posed an immediate risk to the health, safety, and personal rights to children in care.

This poses an immediate risk to the health, safety, and personal rights of children in care. 8 Because the facility has been cited for repeating the same violation within 12 months, an immediate civil penalty of $1000 was assessed. Civil penalty of $100 per pay continues until the violation is corrected.
