Based on the interviews and record reviews, C1 and C2 were left unattended on the playground for approximately five to ten minutes, which poses an immediate health and safety risk to children in care. 8 An immediate civil penalty of 500$ is assessed today.

Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Facilty staff members did not have proof of immunization records.
