Based on the LPA's observations and staff interviews, the sink in Room 4 has been inoperable for approximately one month. This poses a potential risk to the health, safety, and personal rights to children in care.
Based on observation, the licensee did not comply with the section cited above. LPA observed that the rubber foam around one of the structures has holes which are considered tripping hazards. This poses a potential health, safety or personal rights risk to persons in care.
Site Director stated that an All Staff Meeting was conducted on 06/24/24 and reporting requirements was discussed. Site Director stated that she will submit a written plan to ensure that parents are provided a copy of incident/injury report within the required time frame. The facility failed to provide an injury report to parent. This poses a potential risk to the health, safety, and personal rights to persons in care.

Based on record review, the licensee did not comply with the section cited above in 3 out of 7 of the staff (Staff #2, #3, and #5) which poses potential health, safety or personal rights risk to persons in care.

Based on Assistant Director's statement and LPAs observation, the licensee did not comply with the section cited above by serving whole milk to children who are 2 years old and 2% milk to children who are 3 years old and up which poses a potential health, safety or personal rights risk to persons in care.
A child (C-1) was found by a staff member (S-2) unattended in Room 7. This poses an immediate risk to the health, safety, and personal rights of children in care. An immediate civil penalty of $500 was assessed. 8 Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months.
By the end of the POC day, Licensee will complete and will submit a written plan to Community Care Licensing outlining how the facility will ensure that all staff are trained in proper handling of children and ensure that training of future staff are trained to prevent further injuries from inproper handling of Children. Plan of Correction was recieved on the 8/4/2022 of findings delivery. The Facility Staff were not trained in proper handling of children requested immediatly following the first incident by a parent. Training and awareness of staff did not occur until after the second incident. This presents an immediate risk to health and safety of the children in care.
