Based on observation, the licensee did not comply with the section cited above. LPA observed a teacher's aide (S2) supervising 3 infants in the classroom (Area 1) when the fully qualified teacher (S4) needed to use the bathroom located in the hallway near Area 1 which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on assistant director's statement and record review, the licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care.
Based on interviews and available evidence, a child (C-1) fell from the high chair when a staff (S-1) removed the tray attached to the high chair and turned away to place the tray in the sink. This poses an immediate risk to the health, safety, and personal rights of children in care. 8 Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months.
