No child(ren) shall be left without the supervision of a teacher at any time... This requirement was not met as evidence by: 1 Facility representative will create a plan of how they will prevent children being left in classrooms unattented and review it with staff. Licensee to provide proof of having completed plumbing repairs, resulting in operable toilets, to CCLD by POC date. All staff toilets and Shark's classroom toilets are inoperable.

Licensee to provide proof of professional cleaning and mildew inspection having been conducted in areas of previous sewage spillage by POC date. Sewage and mildew odor caused by plumbing issues.
