Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not met as evidenced based on interview and record review. A water outlet at the facility has Director made the affected water outlet inaccessible by covering with duct tape and ceased operation of the water outlet during the inspection. Director stated she will contact a plumber to replace the fixture within two weeks. Director stated she will request a waiver for alternative portable water. very high lead exceedance. This is a potential risk to the health, safety, and/or personal rights of children in care. 8 Director agrees to update Plan of Operation to include corrections made for addressing the lead exceedance. Director stated she will conduct a follow up water sample as outlined in Written Directives section 101705 and submit all required documentation to CCL by 6/24/22.

Based on interview, it was determined facility did not ensure children were accorded dignity in his/her personal relationships with staff by Teacher #2 yelling at children on at least one occasion. This is a potential risk to the health and safety of children in care.
