Licensee has stated that they will move one of the younger infants back to the infant room, bringing the capacity into compliance by 2/6/26.
A REVIEW OF RECORDS WHICH REVEALED CENTER DIRECTOR AMIE MORAGA DOES NOT HAVE THE ORIENTATION CERTIFICATE FOR RECORD KEEPING IN FILE 1 The director contacted Community Care Licensing during the initial visit on 1/30/26 and registered for the record keeping orientation. LPA confirmed. Licensee will submit a copy of the certificate along with a complete director's packet to community care licensing upon completion of the training by 3/5/26.

THIS REQUIREMENT IS NOT MET AS EVIDENCED BY A REVIEW OF RECORDS WHICH REVEALED MS AMIE IS MISSING PROOF OF PERTUSSIS VACCINE, MS. SHELBY IS MISSING PROOF OF MEASLES VACCINE, MS. MICHELLE IS MISSING PROOF OF MEASLES VACCINE AND MS. ANGELINA IS MISSING PROOF OF MEASLES, PERTUSSIS AND FLU VACCINES.
