Based on interview and record review, the licensee did not comply with the section cited above in proof of water/lead testing or attestation was not completed for this facility license and a request for proof was sent on July 24, 2026 and there was no response which poses a potential health, safety or personal rights risk to persons in care.

Based on observation and interview, the licensee did not comply with the section cited above in LPA observed a hole behind the door on each of the children's restrooms in the school aged area which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in acting director has not submitted all of the required documentation to prove their qualifications to be the center director to the Licensing office and the request for the full complete director's packet has been made several times, which poses a potential health, safety or personal rights risk to persons in care.
