101229.1 Sign In and Sign Out (b), the licensee shall develop, maintain, and implement a written procedure to sign the child in/out of the child care center that shall, at a minimum, include the following: (1)The person who signs the child in/out shall use his/her full legal signature and shall record the time of day. 1 During time of inspection Director had parents come in and sign thier infants in. Director sent out reminder to all parents and provied a copy of parent hand book stating parents are to sign in and out thier children. This requirement was not met as evidence by: LPA observed that (2) infants were signed in by staff and not by their parents on the Bright Wheel App. This poses a potential risk to the health, safety, or personal rights of children in care. 8 Under Appeal
1596.8662: Requires all licensed providers and employees to complete the training as specified on their mandated reporter duties and to renew the training every two years. Licensee stated that she did not complete the training. Licensee stated that she will have staff complete on online Mandated Reporter Training (AB1207) and send a copy of the certificate to CCL by 04/10/2024. This requirement was not met as evidence by: LPA observed that (2) staff memebers did not have valid AB1207 Mandated Reporter Training Certificate completed and in file. Director put a thermometer in the refrigerator and throw the food and milk out. During the inspection, LPA observed no thermometer in the refrigerator, the refrigerator door was left open and was unable to verify that the food items were stored at 45 degrees F or less. a thermometer was placed for 30 minutes and the temperature did not reach 45 degrees. This poses a potential risk to children in care.
Director labeled the breast milk with name of child and confirmed with staff the date it was stored. During the inspection LPA observed breast milk was in the infant freezer with no label, it did not have a child’s name or date it was stored. This poses a potential risk to children in care.

LPM's observation of 1 Per licensee, she will provide training with staff to review requirements of infant care and supervision. LPM & LPA Observing 2 1 Licensee stated that they will conduct a 1 on 1 staff training for staff regarding ratio coverage. Licensee stated they will provide Community Care Licensing with proof of documentation of training. teachers caring for 16 infants alone. 6 Infants were awake while 10 infants were asleep. Eunice stated that the 3rd staff left for lunch without obtaining coverage and permission from director. This poses a potential risk to the health, safety, or personal rights of children in care.

Based on observation, LPA observed Child #8 and Child#10 (both infants) commingling in main lobby with both preschool and school age children on 11/17/2022. This poses an immediate risk to the health, safety, or personal rights to children in care. 8 create an action plan for morning drop offs that ensures that every child is being monitored by a qualified Staff at all times with the appropriate License capacity and following Title 22, Division 12, Chapter 1 regulations and submit it by 01/05/23.
During record review it was noted that Staf #3 did not have a Criminal Record Clearance. This poses an immediate risk to the health, safety, or personal rights to children in care. Civil penalty of $100 has been assessed. 8 Staff #3 left during today's inspection and cannot return until fingerprint clearance has been granted. Licensee understands that Staff #3 can work in faciltiy, with children, upon clearance.

Based on record review, the licensee did not comply with the section cited above in five out of seven children's records did not contain a physician's report which poses a potential health, safety or personal rights risk to persons in care.
Based on observation and record review, the licensee did not comply with the section cited above in Child 1, who turned two years old in June 2021, was being cared for in the infant classroom which poses a potential health, safety or personal rights risk to persons in care.
(1) An aide may be substituted for a teacher when all of the following conditions are met: (A)There is a fully qualified teacher directly supervising no more than 12 infants; and (B) Each aide is responsible for the direct care and supervision of a group of no more than four infants. This requirement was not met as Licensee stated she shall submit a written statement detailing how she will ensure that qualified staff are present in the infant room at all times. The written statement shall be submitted to CCL by 09/22/2021. evidenced by: Based on observation and record review, an aide was providing care and supervision to seven infants and the fully qualified teacher had more than 12 infants in care. This poses a potential risk to the health, safety, or personal rights of children. 8
