Based on multiple witness statements, LPA learned that 1 Facility Representative (FR) stated the classroom has been rearranged to prevent blind spots so staff can visually supervise all children at all times. FR also stated staff have received additional training and additional observation and guidance from management in the facility. inappropriate incidents between children occurred due to lack of visual observation by staff. This was a self-reported incident. This poses an immediate threat to the health, safety, and/or personal rights of children in care. 8 FR has provided LPA with documentation of the training and showed LPA the changes in the classroom. The POC has been met.

Staff training was conducted immediately following the incident in order to prevent this type of situation in the future. LPA reviewed memo that staff received. LPA requested that the memo was signed by each staff and submitted to LPA via email by

LPA observed a preschool classroom transitioning to nap time with 13 children and one staff. All children were still awake. 1 Director stated if they have extra staff they will send someone in to assist getting children to sleep. Director stated she will create a staff memo letting staff know that they cannot leave for lunch until all children are napping and staff will sign the memo.
