Responsibility for providing Care and Supervision. The Licensee shall provide care and supervision necessary to meet the children's need.. Supervision shall include visual observation. This requirement was not meet as evidence by: 1 Director will conduct a staff meeting/training to remind staff to check blind spots and ensure to have visual supervision at all time and complete the CSR log. Director will send picture of dipper area door removed and proof of training log to LPA via email by 06/23/25. Staff not having visibility of child when being in dipper changing area (blind spot.) Based on observations today it was evident that changing station door creates a blind spot and door lock is accessible to children. 8
