Based on observation the Licensee did not comply with the requirement above as toilet and sinks in classroom were covered in debris which poses a potential health, safety or persona rights ricks to children in care .
Based on record review, the facility did not comply with the section cited above in that Staff #1 was missing Pertussis and Flu information, Staff #2 was missing Measles and Petussis information and Staff #3 was missing Pertussis information. This poses/posed a potential health, safety or personal rights risk to persons in care.
Based on record review, there were 11 children who were present did not have currently updated Infant Needs and Services Plans on file. This poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation, the facility did not comply with the section cited above in that there was no protein served for the lunch meal. It consisted of rice, green beans and salad with dressing. This poses/posed a potential health, safety or personal rights risk to persons in care.
