Based on LPA observations and staff interview statements, there was 1 teacher with 6 infants in infant room A and 9 infants in infant room B. 1 The Director shall ensure that either herself or another teacher will monitor and fill in when necessary to maintain infant/teacher ratio. A staff meeting discussing ratio requirements and a declaration signed by Director shall be submitted to LPA by 3/17/2023. This poses an immediate Health and Safety, and personal rights risk to persons in care.

On 06/27/2022, Child's needs were not met when staff failed to properly supervise Child during water play. As a result, Child's wound and bandage got wet when 1 Assistant Director stated that facility will revise School's Temporary Exclusion Policy under Admissions Agreement and Family Handbook and email it to LPA Martinez no later than clsoe of business on Friday, October 21, 2022. Child dipped hand in water table. This is a Type B citation and poses a potential risk to the health and safety of children in care.
