Based on record review, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care. LPA observed 2 staff are missing the Mandated Reporter Certificate

Based on interviews, licensee did not ensure that child in care was accorded dignity in their relationship with staff. Child #1 was allowed to kiss Staff #2 on cheek. Staff #2 was not positoned in a way suitable to appropriately prevent incident. This poses a potential risk to the health, safety, and personal rights of children in care.

Staff #4 admitted that the incident report dated 10/27/2021 was not given to the parent during pick up. Evidence has revealed that 2 out of 16 incident reports were not signed by parent/guardian, one was signed the following day. Three of the incidents required administration of first aid by staff. This poses a potential health and safety risk to the children in care.

Evidenced have revealed that staff have told a child that the child will not get a cupcake if the child did not lay down and take a nap.
