Based on the self-reported unusual incident report, and staff interviews, it was determined that the staff #1 (S1) fed the wrong bottle of breast milk to Child #1 (C1) that belonged to Child #2 (C2) whiich poses a potential health, safety or personal rights risk to the persons in care. 8 CCR

(1) No child(ren) shall be left without the supervision of a teacher at any time.... Based on interviewed and self-reported unusual incident report, C1 was given the incorrect milk bottle during feeding time for a few minutes, in which, C1 drank 1 to 3 ounces of formula milk. This poses and immediate health and safety risk to children in care.
Personal Rights- (a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. 1 On 03/12/2022 new health order took effect and mask are no longer required, but strongly recommended. No action needed at this time. Type b deficiency cleared during today's visit. This requirement was not met as evidence by interview with director and documents provided of not requiring staff and parents to wear mask while at the facility on 03/09/2022, 03/10/2022 and 03/11/2022. 8
