This requirement is not met evidenced by: 1 DIrector stated they had one on one meetings regarding active supervision, constantly walking around, and observation. Based on the unexplained injury to C1 head, interviews with staff stating they were unsure how the incident occurred, parent interview, pictures of the open wound laceration on C1 head, and doctors after visit summary report. This is an immediate risk to the health and safety of children in care. 8

Assistant Director will email LPA by July 10, 2023 confirming that the new teacher has started working in classroom Discovery P1 classroom. This requirement is not met as evidence by: Based on LPA and LPM interviews with S1 and S2, it was determined C1 was left in the classroom alone for approximately 1 minute. Director agreed to send a copy of agenda with staff signatures to LPA POC due. This requirement is not met as evidence by: Based on LPAs interviews with Director and S1, it was determined C1 was left on the play yard alone for approximately 8 minutes and was discovered by a parent from the parking lot entering the facility. This poses an immediate safety risk to the children in care. 8

As a result of this incident Staff #1 is no longer working at the facility. A staff meeting discussing supervision and ratio will be conducted on 11/24/21. Proof of these items listed will determined that on 10/19/21 a Staff #1 left a child #1 and child #2 on the outdoor play area for approximately 5 to 7 minutes. This has been determined to be an immediate threat to the chidren's health, safety, and personal rights. 8 be provided to LPA today on 11/17/21.
Based on LPA's interviews it was determined that on 10/19/21 a staff #1 took 13 preschool children to the outdoor play area, for 1 In response to the incident the director met with each staff individually the next day on 10/20/21 and discussed the incident and supervision/ratio compliance.
