Based on staff interview staff could not provide documentation supporting whether the aide had qualifying educational units to aide a fully qualified staff.
Subsequent to initial licensure, a person specified in subdivision (b) who is not exempt from fingerprinting shall obtain either a criminal record clearance or an exemption from disqualification, pursuant to subdivision(f) of this section or Section 1522.7, from the State Department of Social 1 LPA observed S1 providing Care and Supervision to the children in care. S1 has been employed since August 2025. Upon reviewing the facility association roster, it was discovered S1 does not have a Criminal Record Clearance. $500 Civil Penalty assessed. Based on the observation and record review, the Licensee did not meet the above regulation which poses an immediate risk to the children in care. During the facility tour the 8 Director understands all staff providing Care and Supervision must have a Criminal Record Clearance prior to working, residing or volunteering. Director agrees to have S1 obtain a Criminal Record Clearance. Documentation must be submitted to the Department by COB 5-29-2026.

Based on the interviews and records reviewed, the Licensee did not meet Staff-Infant Ratio which poses an immediate Health, Safety & Personal Rights risk to the children in care. During interviews with pertinent parties it was disclosed that staff have been out of ratio on more than one occasion.
(e) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility:(1) Obtain a California clearance or a criminal record exemption as required by the Department or(2) Request a transfer of a criminal record clearance as specified in Section 101170(f). This requirement was not met as evidence by: Staff/Director agrees to verify that all employees are associated before starting working at the facility. Civil Penalty assessed. $100 per day for 5 days per person. Written statement of understanding was providing by facility director during this inspection. Based on the interview/record review, the Licensee did not meet Criminal Record Clearance which poses an immediate Health, Safety & Personal Rights risk to the children in care. During the inspection LPAs observed S1 and S2 providing care & supervision. Staff 1 & 2 do not have a Criminal Record Clearance associated to the facility. 8
