Deficiencies cleared during inspection. Health, Safety & Personal Rights risk to the children in care. During record review the facility self-reported that Child # 1 was given Child # 2’s bottle. Pertinent parties confirmed this to be true during interviews. Per Child #1’s Infant and Toddler Schedule he/she should have been given Similac but was given an Enfamil.

This requirement was not met as evidence by: 1 Director agrees to conduct in service training regarding sueprvision and will provide CCL with training date and a draft of the topics and tto be discused by 8/25/2022. The Director also agrees to provide a sign in sheet from the day of training. $500 Civil Penalty assesed. Based on the interview and record review, the Licensee did not meet the regulation which poses an immediate Health, Safety & Personal Rights risk to the children in care. During record review the facility self-reported Child #1 walked away from his/her classroom. During and interview it was confirmed there was an absence of supervision by the Director. 8
