Based on LPA observation, records reviewed and interviews conducted, Facility staff could not recall changing or toileting the child from 4:07pm until 5:33pm. Additionally, staff did not log any diapering changes or potty training attempts in the Kindercare app. This is an immediate risk to the health and safety of children in care. 8 Director agrees to submit training agenda and training sign in sheet on to LPA by POC date of 02/14/2023.

On 10/26/2022, Assistant Director provided a copy of Release for a Personal Care Assistant or Service Provider form. Per Assistant Director, the form is in the process of being completed. Assistant Director/Director agrees to have the form fully completed and agrees to submit a copy of the form to the Riverside Regional Office. During facility observations, LPA Hogue observed an One on One Aide assisting a teacher in the Three's Classroom. The Aide was setting up classroom mats, serving milk, and handing out blankets to children in care which poses a potential health, safety, and personal rights risk to children in care. 8 Assistant DIrector agrees to place the completed form in the child(ren)'s file. Assistant Director agrees to submit a written plan detailing how the facility will ensure they're following KinderCare's Plan of Operation, specifically for a child(ren) needing an One on One Aide or any other individual services.

According to interviews, the child(ren) was in the classroom unsupervised for approximately one to two minutes. Assistant Director agrees to submit staff meeting and training agenda along with staff meeting attendance sheet. A CIVIL PENALTY WAS ASSESSED DURING THIS INSPECTION.
