Based on observation, LPAs observed that there was a cabinet under the changing table that had spray bottles with cleaning supplies that was not locked. There was also a cabinet in Discovery Preschool B Room that had spray bottles with cleaning supplies that was accessible to children. This poses a potential health and safety risk to children in care. 8 in the Discovery Preschool B room to a locked cabinet.

No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement is not met as evident by: 1 By POC 02/01/2023, Director will conducted training with staff and send the list of staff who received training. Based on interview, C-1 was in the classroom by themselves, which poses an immediate risk to children in care. 8 Director submitted agenda for training during today's inspection. A civil penalty of $1000 was assessed.

This requirement is not met as evident by: 1 S-1 and S-2 are no longer working at the facility. By POC 11/10/2022, Director will submit plan on how they Based on interview and record reviews, S-1 and S-2 did violate children's personal rights, which poses an immediate risk to the health and safety of children in care. 8 will ensure that children's personal rights are not violated. Based on observation, LPA observed that there were two children in the bathroom. There were no staff standing where they could see the children in the bathroom, wihich poses a potential health and safety risk to children in care. 8
