Plan of correction was completed prior to 10/01/2024 investigation inspection. LPA verified correction at that time. Infant ingested a piece of a rubber glove while in care, this poses an immediate health and safety risk to children in care.
Reyna states that an additional carpet will be purchased to be used for the non-mobile infants to crawl on while in the big toddler room at the end of the day. Carpet shall be purchased and proof will be submitted to the Department by 5/15/2024. The staff did not ensure that the carpets were clean for crawling/non-mobile infants as they are placed onto surfaces where mobile infants and staff are walking and observed to have visible debris, which poses a potential risk to the health, safety, and personal rights of children in care.
The Site Director states that she shall change the route of how to get the infants back into the classroom. The infants shall go through the toddler classroom and out of it instead of going outside of the preschool yard. The Licensee did not ensure that the preschool and infant components were physically seperated during transition from outside play to inside play, which poses an immediate risk to the health, safety, and personal rights of children in care. 8 The Site Director will submit a map indicating the route that the infants will take when transitioning inside and will have it signed by the teachers who are responsible for those children by end of day 3/15/2024.
Based on interviews and record reviews, LPA learned that five staff do not meet the minimum requirements for qualifications.This poses an immediate risk to the health, safety and personal rights of children in care.
(b) There shall be a ratio of one teacher for every four infants in attendance. Assistant Director will submit a written plan of correction by POC Date of 01/11/2024.
