Due to interviews and documentation gathered, it was determined that children in care were not assisted during restroom times and children were sent home with dry fecal matter on their bodies, which does not meet hygenic standards and regulations. Based on complaint allegation evidence collected, the Licensee did not ensure that children were given dignity in their personal relationships with staff as S1 is yelling at children, which poses an immediate risk to the health, safety, and personal rights of children in care. 8 S1 shall also enroll into a training regarding topics such as redirection, discipline strategies, self-regulation, toddler development (working with two year olds) etc. and submit proof of registration to the Department by 5/23/2024.
The Site Director shall submit proof of signed LIC9224 for all children enrolled into the preschool program by 5/31/2024. The facility shall submit POC by 3/28/2024 for program administrator to complete additional units and be fully qualified as she does not have the units required to step in for fully qualified staff. The Licensee did not ensure that staff are not with more than 12 preschoolers at one time as classroom #3 was observed with twenty four children, staffed with one fully qualified teacher and one qualified aide, which poses an immediate risk to the health, safety, and personal rights of children in care.
LPAs provided list of children who need physicians report completed to the Assistant Director and requested that the completed LIC701 is submitted to the Department by 4/26/2024. During review of children's files, LPAs observed that three out of twenty one files did not contain a physicians report (LIC701) prior to enrollment or within thirty days following enrollment, which poses a potential risk to the health, safety, and personal rights of children in care.

During review of classroom #4 files, LPAs observed twenty out of twenty one files did not contain signed verification (LIC9224) of report from 3/14/2024 regarding Type A citations, which poses a potential risk to the health, safety, and personal rights of children in care. 8 Scanned copies of signed reports (LIC9224) shall be submitted for all preschool parents to the Department by 4/3/2024.
The Licensee states she will provide a smaller waste basket for the children, rearranging the food area for the children to make it more contained, and for diaper changing will retraining on appropriate diaper changing routines more 1:1 to ensure they understand. The Licensee did not ensure that the children were given safe and healthful accommondations during meal time, in providing diaper changes, and in maintaining nap mats, which poses an immediate risk to the health, safety, and personal rights of children in care. 8 Will discuss with staff about transition to nap time and wait for all to be ready to all move to their nap mats. LPAs advised that canvas bags may be used for storing nap materials as long as they have individual names.
