Based on interviews conducted and records reviewed, staff did not observe child 1 (C1) being kicked in an incident which occured on 8/23/24 which poses a potential health and safety risk to children in care. 8 Director stated she will review the forms during a staff meeting on 9/19/24 and email LPA Selena Mariani at selena.mariani@dss.ca.gov, proof of staff attendance.

Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs. This was not met as evidence by. . . Assistant center director reported since the incident had occured, the center had conducted one on one training with the staff. Center agreed to take training on active supervision. Based on interviews conducted, staff were present in the classroom and did not witness the choking. Also Staff did not prevent the situation from escalating. This poses a potential health and safty risk to children in care. 8 And email, mail or Fax LPA Hernandez Torres, attendance sheet of the staff who participated in the active supervision training.

Based on obsevation and record review all three staff (S1-S3) were working in the capacity of a teacher under the school age license. S1-S3 did not provide the qualifications to be left alone with children as a teacher. Center director stated that she will reach out if TSP is the route to take. . . . as evidence by, based on interviews and observations staff at the center are not mitigating verbal and physical bullying.
