Based on record review the licensee did not comply with the section cited above due the trainings not have the EMSA approved stickers which poses/posed a potential health, safety or personal rights risk to persons in care.
(1) In a safe condition for the activities planned.(2) Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. Director stated he has put in a work order to have the ground leveled and resurfaced and that area of the yard is not being utilized until the changes have been made to free the space of hazards. This requirement is not evidenced by: Based on observation, interview, and record review the facility did not meet this requirement by having hazards of holes and uneven ground in the outdoor activity space 8
Based on observation this requirement was not met by having 18 preschool children with 1 teacher. 8 Retraining to staff will be provided with ratio requirements and supervision requirements, signed and dated by all employees and provided to LPA. Additional staff is hired (LPA witness a new employee).
Based on interview and record review this requirement was not met by not maintaining full supervision due to staff shortages.
(a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance 1 Director Robb Staples stated he has hired two new staff to maintain ratios in the future.
