Facility Representative (D1) stated usage of the outlets was stopped upon receipt of testing results. Facility will use a different classroom with a water outlet that tested within requirements. LPA observed 15 children in care with one staff. This is an immediate health and safety risk to children in care. 1 Director stated she will address the issue with the teacher today verbally. A self-reported incident and statements made in interviews revealed that Child #1 was left unattended outside for 15 to 20 minutes without supervision. This is an immediate health and safety risk to children in care. 8 Civil penalty in the amount of $500.00 is assessed for absence of supervision.
Information was received from facility that Staff #1 squeezed and scratched Child #1’s hand. Child #1 1 Assistant Director stated Staff #1 has been fired and LPA did not observe them on the premises. LPA advised that it is best practice to have more than one staff in a classroom to ensure accountability.
