Based on interview and record review, the licensee did not comply with the section cited above. Staff failed to provide visual supervision on 11/15/2024 during transition, a 2 year old child was left in the classroom while the rest of the children transitioned to the outdoor play area. According to staff, child was without visual supervision for approximately 2 minutes. This posesd a potential health, safety or personal rights risk to persons in care.

… 1 Director stated that she would provide a staff training regardfing observation of the child and when to report incidents to parents regarding unusual events and email it to: Keturah.Lane@dss.ca.gov by 6/24/24. Based upon interviews with parent and staff members, and medical records received, C1 fractured her elbow during morning play, and C1 was not continually observed throughout the day, nor parent contacted by staff members which posed an immediate health, safety and personal rights risk to children in care.
There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below. This requirement is not met 1 Director stated that she will tallk to staff members to remind them of the Kindercare name to face head count to ensure that the all staff member is aware of the number of children that they are supervising. as evidence by: There were 13 children from the Preschool Classroom that were at the playground supervised by staff Lauren Digiacinto. Staff thought that she only had 12 children. This poses a potential safety risk to children in care. 8
All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: 1 Facility representative completed the criminal background clearance transfer form and submitted the require transfer documents to the Licensing office via email during today's inspection. Request a transfer of a criminal record clearance as specified in Section 101170(f). This requirement is not met as staff Andrehya Azzato has been working at the facility for over a year and her fingerprint is not associated to the facility. This poses a potential health and safety risk to children in care. 8
Based on observation and interview with the Director, the licensee did not comply with the section cited above as the facility had recently replaced the play structure at their preschool and pre-k playground. The construction started in May and it was completed at the end of June of 2023. Facility did not notify licensing prior to the alterations. This poses/posed a potential health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the directive above, as one out of seven outlet (outlet used for drinking) tested with an Action Level Exceedance (ALE). This poses an immediate Health and Safety risk to children in care. 8 testing company. Facility has been using the other water faucet to prepare food and drinks for the children in care.
Based on record review, the facility did not comply with the directive above, as three out of seven outlet tested with an Action Level Exceedance (ALE). (both drinking fountains has been shut off since COVID). This poses a potential Health and Safety risk to children in care. 8 Facility has replaced the kitchen faucet and the pipeline since the facility obtained the result. Facility will not use the kitchen faucet for cooking or consumption until it gets clearance from the water testing company. Facility has been using the other water faucet to prepare food and drinks for the children in care.
