.... 1 Correction has already been addressed. Per Ms. Howerton, all infant staff has been retrained in “Safe Bottle Feeding” as of 1/22/26. LPA was provided with training agenda (with staff signatures), which included enhanced sign-off requirements. Based upon facility documents and interviews with multiple parties, Child C2 was given Child C1's bottle to drink. This posed a potential risk to the health and safety of the child.

... 1 Director stated she would review the trainings with staff on an on diaper changing procedures by 5/10/24. Director will send staff sign-in sheet and agenda via e-mail to LPA Lane. Based upon Kindercare app and RP interview, C1 was not changed often enough (4 hours between changes) which may have caused a rash and is a potential health, safety and personal rights risk to children in care.

All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: Request a transfer of a criminal record clearance as specified in Section 101170(f). 1 Director submitted Maryam Mosa criminal background transfer request to the licensing office via email during the inspection. A copy of the facility's associated list was provided to the Director today. This requirement is not met as staff Maryam Mosa has been working at the facility since August of 2023 and her fingerprint is not associated to the facility and a fax confirmation receipt is not available for review. This poses a potential health and safety risk to children in care. 8 Director stated that she will review the association list to ensure all staff members are associated prior to working. She will also have the fax confirmation receipt in the staff's file or proof of email send to licensing. Director will submit a written plan of correction to LPA no later than 11/06/2023.

Based on observation and interview with Director, the licensee did not comply with the section as facility had replaced their play sturcture at the toddler playground that started in May of 2023 and completed on June 2023. There shall be a ratio of one teacher for every four infants in attendance. This requirement is not met as evidence by: On 1/3/2023, facility was operating out of ratio for about five minutes in the morning per review of records. 1 Director stated that she adjusted the staff members hours by having another staff takes over the shift of the staff that was struggling to be at work on time during mid January 2023. One teacher supervising five infants as the fifth child was dropped off at 7:20am and the second staff did not arrive to class until 7:25am. Statements obtained, there has been times that the room was out of ratio due to a staff being late to work. This poses a potential health and safety risk to clients in care. 8 Director will remind all infant and toddler staff to ask the parent to stay or seek help to ensure that the facility remains in ratio at all times. Training agenda and staff members sign in sheet shall be submitted to LPA by 3/14/2023 .

Child care center personnel shall be competent to provide the services necessary to meet the individual needs of children in care and shall at all times be employed in numbers sufficient to meet those needs. 1 Director conducted training with all of the infant teachers and support teachers regarding the name to face check before placing the bottle in the warmer and before feeding the baby with verification from a second This requirement was not met when Staff #2 and #3 fed the wrong bottle of milk to infant #1 because they did not verify the bottle information prior to feeding. Infant#1 did not sustain any allergic reaction to the wrong bottle of breast milk. This is a potential hazard to children in care if not corrected. teacher per KinderCare's bottle policy. Director submitted the staff's members training agendas with all the staff's names and signatures of the training provided to them.
