This is a potential health and safety risk to the children in care.

Based on interviews conducted 3 of 4 staff members stated that they will be out of ratio in the mornings for up to 10 minutes and based on record review LPA observed on 5/28/25, 5/29/25 and 6/3/25 classroom to be out of ratio. This is a potential health and safety risk to children in care.
Based on 6 of 6 staff and confidential interviews that C1 had been having behavioral challenges since prior to 1/2025 when the old Director left the facility. However, based on record review and interviews facility did not contact the Inclusion Department for assistance until 3/2025 and then soon after the child dis-enrolled. This is a potential risk to the health & safety of children in care.
Based on 6 of 6 staff and confidential interviews that at least 7 preschool children were being moved from their prek classroom to the kinder/school-age classroom due to the children being older prek and to assist with behaviors, despite the children not being of age to advance to the school age program. This is a potential health & safety risk to the children in care.
Based on record review, the licensee did not comply with the section cited above in 2 out of 4 persons missing updated Mandated Reporter certificates in file which poses/posed a potential health, safety or personal rights risk to persons in care.

Director stated all faucets/fountains have been replaced and facility is in the process of scheduling retesting. Director will submit new facility sketch, LIC 9275, LIC 9276 and results of retests to LPA Hamilton. LPA Hamilton received water testing results taken on 10/08/2022 with an Action Level Exceedance which is higher than the allowable limit. The faucet is located in Pre-K classroom and an empty classroom.
