Based on observation record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. LPA observed 4 staff member with an expired mandated reporter training certificate. Director stated that mandated reporter training certificate is normally done in August, but due to the covid closure which put the facility off training schedule.

LPA observed staff did not have LIC 503 (health screening) Director Stated that LIC 503 (health screening) might have been missing or missed placed.
