Based on observation, interview, record review, At 3:45pm while reviewing children’s files 2 out 5 children’s files were missing immunization records. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
During visit, Director associated three staff via Guardian website. Staff with "PENDING" status cannot return to facility until overall status has been cleared eligible to work. A review of the Facility Personnel Report Summary on 09/21/2022 indicates three facility staff were not associated to the infant license and one associated staff status is "PENDING". This is an immediate risk to the health and safety of children in care.
Based on observation and interview with Director, one child was missing signed in/out with full legal signatures. This is an potential risk to the health and safety of children in care.
