101223(a)(3) Personal Rights (a) The licensee shall ensure ...personal rights: (3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion...functioning. This requirement was not met evidence by: 1 Director stated the children in room 4 can pick whatever areas they want to play at when combined in room 2. Director stated she held a meeting with all the staff. Action Plan and staff sign- in sheets will be sent to LPA by POC date. Based on interviews and observations conducted by LPA, it was determined that staff limited the children to table top activities and the small carpeted science area for the extended periods of time when combined in room 2. This is a potential Health and Safety risk to the children in care. 8

Based on interviews and records obtained S1 raised their voice at C1 during lunch time and S1 placed their foot on top of C1's body during nap time. This is an immediate risk to the health and safety of the children in care.
