Based on LPA Wheatley's observation, interview and record review, the licensee did not comply with the section cited above in that three staff members (cook, piano teacher, art teacher) are not fingerprinted or live scanned. LPA called the El Segundo office and verified that the staff do not have a fingerprint clearance. The cook is around the children daily. The piano teacher was supervising children alone today. The art teacher is here once a week however she teaches the children piano lessons without a fingerprint clearance. This poses an immediate health, safety or personal rights risk to persons in care.

Based on LPA's observation, and record review, the licensee did not comply with the section cited above in that 4 out of 5 employees do not have the required Mandated Reporter Training.
