101229Responsibility for Providing Care and Supervision.(a)The licensee shall provide care and supervision... to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time,. Supervision shall include visual observation.This requirement is not met evidenced by: 1 Facility Representative stated, she will talked to the staff regarding supervision, new staff members will trained with Director, assistant Director, assigned to lead teacher that will train them in classroom setting. Director will conduct meeting with the staff, agenda will be sent to LPA poc date. Staff did not observe how C#1 left the basketball court and was observed on the upstairs walkway, where door is with push bar and leads to the parking lot, which posses potential risk to the health and safety of child in care. 8

Based on observations, records reviewand interview, the licensee did not comply with the section cited above. During the file review, LPA observed 3 out of 12 files were missing records of immunizations. S#12 missing Measles, S#4 missing Pertussis, S#3 missing Measles and Pertussis, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observations, records review and interview, the licensee did not comply with the section cited above. 2 out of 12 staff files were missing LIC 503 forms. S#3 and S#2 were missing Health Screening Forms, which poses/posed a potential health, safety or personal rights risk to persons in care.were

(d) The surface of the outdoor activity space shall be maintained:(2)Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. Facility Representative stated bike yard temporary will not be in use, other yards will be used on the stagered schedule. Cracks will be temporary filled and new pavement will be placed by begining of the new year.

Based on observation, interview, record review, the licensee did not comply with the section cited above 1 out of 8 children does not obtain a completed LIC 613A Personal Rights from for LPA to observe, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, record review, the licensee did not comply with the section cited above in 1 out of 7 staff did not obtain proof of completed Infant/Toddler course for LPA to observe, which poses/posed a potential health, safety or personal rights risk to persons in care.

Testing results with fractional ppb readings of 5.5 ppb or greater shall be rounded up to the nearest whole number...(1) A result with values of 5.5 or greater shall be deemed and Action Level Exceedance This requirement was not met as evidence by: Per Facility Representative facility will come in agreement with the church and either remove the facet or replaced it and redo testing. Facility had a lead exeedance reported to the Department. Facility had a 3 faucets with the lead exceedance. 8
