Based on record review, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.
Licensee shall provide additional training to staff regarding children's personal rights and provide proof to the department by the POC date. Video footage reviewed showed C1 lying down on a cot belly down with their head up. Staff approached child and used mild force to get the child to fall asleep.
Water testing results identified with Action Level Exceedance as defined in WD section 101700.3 are not deemed safe to drink. This poses a potential Health and Safety risk
LPA observed 29 children in 1 Per assistant Director, will rearrange staff schedules to have them come earlier. Facility will submit copy of staff schedules to indicate sufficient staff in place to ensure compliance. Will submit by Monday 6/26/23. the Pre-k room supervised by 2 staff. This poses an immediate risk to the health and safety of children in care. Civil Penalty assessed as this is the 2nd citation within a 12 month period.
At 8:40am, upon entry of the Pre-k classroom, LPA observed 1 staff was supervising 25 children. This poses an immediate health and 1 Per director, 2 staff called out today. Director called 2 staff and 2 additional arrived during visit. Per director, will rearrange staff schedule to have staff come early to ensure compliance. Deficiency corrected during visit. safety risk to children in care.
