Director stated that staff schedules will be revised to ensure sufficient coverage to meet children's needs, particularly during nap time and outdoor activities. The Director will meet with staff to discuss strategies to support children's potty training goals and proper supervision. More than 50% of people interviewed disclosed that children’s toileting needs were not being met and that there was supervision concerns due to all the changes in staff. 8 Meeting notes with staff signatures and revised staff schedules will be emailed to LPA Macias by 08/14/2026.

Based on LPA interviews conducted with staff, parents, children and third party interviews, it was determined that staff engaged in conduct that included yelling at children and treating them with disrespect. Additionally, one incident of improper physical handling of a child was directly observed. These actions pose a potential health, safety, and personal rights risk to children in care. 8 Director stated that staff will attend training and sign a statement on understanding of children’s personal rights policies. Director stated that Agenda, and signed staff attendance sheet will be provided to licensing by 7/06/2026.

Executive Director stated that an Unusual Incident Report will be turned in by 06/05/2026 and a signed and reviewed copy of Title 22 regulation 101212. On 3/18/2026 an incident occurred where an infant was fed another infant's bottle with breatsmilk, and the incident was not reported to licensing and an Unusual Incident Report LIC 624 was not received. This poses a potential health, safety or personal rights risk to the children in care.

Executive Director stated that besides the labeling, a new system was implemented and teachers were trained. Executive Director stated that signed by the staff instructions on new system will be emailed to LPA Macias by 06/05/2026. On 3/18/2026 an incident occurred where an infant was fed another infant's bottle with breatsmilk. Per documentation and staff interviews it was found that the infant was given the wrong bottle. It is unsure if the infant drank the breastmilk. This poses a potential health, safety or personal rights risk to the children in care.
