Based on record review, the licensee did not comply with the section cited above in seven counts out of 12 counts of staffs files finding expired certificates of mandated reporter training which poses potential health, and safety risk to persons in care. Staff # 1, 2, 3, 7, 10, 11, and 12 missing renewed certificates.

This is an immediate risk of personal rights to the children in care. 8 Facility has implemented a new thorough employee hiring process that is conducted by the Executive Director, Alexandra Magliarditi. All staff will complete retraining of Mandated Reporter training by 01/03/2023. Director will email staff completion certificates by 01/04//2023.

Director will submit a written statement with plan to correct ratio deficiency which includes but is not limited to changing timeframe of scheduled meetings. Director will email LPA statement at romelia.castanon@dss.ca.gov by specified date on 12/15/2022. LPA observations on 10/26/2022, interviews with Staff #1, #2, #3 and #4, documentation written by former director stating facility was out of ratio.
