Based on a complaint report, interviewing 8 staff, reviewing name to face sheet dated 6/17/2026 that one staff member 1 The director agreed to send a statement with a plan of correction, and a correct staff communication to show that the preschool classrooms are staying in ratio at all times. Staffs' signatures acknowledging being aware of teacher child ration and how was left alone with 20 preschool children from 8:05 to 8:37am. Although S2 was present in the classroom from 8:00am to 8:15 am, S2 had not clocked in, observing a school project, and not supervision children. Additionally, S4 was entering and exiting the classroom. There is no record of signing S4 on the name to face sheet. Therefore, the facility failed to maintain the required preschool teacher child ratio of 1:12 between 8:05 to 8:37 am on 6/17/2026. This is a potential risk to the health and safety of children in care. 8 to communicate for help in these situations will be sent to LPA's email address by the due date of 7/13/2026.

This requirement is not met as evidence by: Based on self-report UIR and staff interview conducted on 4/12/24, C1 was left alone without any adult supervision. This pose an immediate risk to the health of the children in care. 1 The Center Director in charge stated that S3 had been terminated regarding the procedure for absent calls. Director agrees to send a plan of correction for how this incident will be prevented in the future. The plan of correction will be sent by the due date. Based on interviews and statements from Director and HR, S3 failed to supervise a child from Pre/K to T/K room. This poses an immediate risk to the health and safety of the children in care.. 8

Personal Rights(a)(3) The licensee shall ensure that each child is accorded the following personal rights: To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat....aids physical functioning. Director terminated S6 on April 24, 2023 due to incident that occurred at facility. On 7/14/23 Director provided LPA with a written statement, stating she will conduct a training for all staff by 7/21/23. This requirement is not met as evidence by: Based on LPA’s interview, S1 disclosed witnessing S6 tapping child with the broom on the child’s head and S6 was going to smash the broom over the child’s head and would hurt the child with the broom. The child was crying. This action poses an immediate risk to the health and safety to the child in care. 8 Director Mary Joy Hilsabeck will conduct a Personal Rights training with all staff and will provide LPA Duron with training sign in sheet by 7/21/23.
