Based on interviews and record reviews the facility did not comply with the section cited above when LPA conducted a visit on 11/13/2025 and .... 1 By 12/12/2025 Close of Business, the facility will submit a written statement to LPA, explaining how the facility will remain in compliance. LPA was also informed that facility is currently in the process of interviewing for a Director. LPA was told that S1 was an "Acting Director" until the position was filled, which S1 later informed LPA that they were just a Teacher and the facility currently did not have a Director. During this period multiple incidents have occured which poses an immediate risk to the health, safety or personal rights of children in care. 8 Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Based on observation, interview, and record review, the licensee did not comply with the section cited above by not ensuring all staff have an eligible criminal background clearance prior to working in the facility, which poses/posed a potential health, safety or personal rights risk to persons in care. Bouchra Chahboune did not have an eligible criminal background clearance during today's visit. The Administrator was informed to check Guardian or contact the Oakland office to very clearance.

This requirement was not met as evidence by: 1 By 10/06/2023 submit a written statement on how the facility will never leave a child without supervision of a teacher. ex: Have more staff in the morning. LPA observed the children in the classroom alone, while staff was in another classroom on two separate occasions, which is a potential risk to the health, safety, and personal risk to the children in care. Both times, the staff immediately returned to the classroom. 8
