Based on interviews, the facility did not comply with the section cited above when C1 and C2 were left unsupervised in the "ladies dressing room" area off the green room on multiple occasion which resulted in a physical altercation between the two children which poses an immediate risk to the health, safety or personal rights of children in care. 8 Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.
Based on interviews, the facility did not comply with the section cited above when LPA's were informed by current and former staff that preschool aged children and school aged children were conmingling more than three times a week which poses an potential risk to the health, safety or personal rights of children in care.
