Based on record review, the licensee did not comply with the section cited above in staff #4, #6 and #8 lack Health screening form and TB test which poses/posed a potential health, safety or personal rights risk to persons in care.
Director confirmed that the faciilty did not have running water the week of 9/26/2022 due to a Per Director, the leak was fixed on 9/30/22 in the early evening. The facility was closed on 9/29 & 9/30/22. Deficiency has been corrected. leak. This poses a potential risk to the health and safety of children in care.
Based on observation and review, the licensee did not comply with the section cited above in LPA observed child #1 & Child #4 Epipen expired which poses/posed a potential health, safety or personal rights risk to persons in care.
