(2)To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. 1 Director and Licensee will hold an all staff meeting & view the personal rights video and discuss the importance of level of care, and submit a written action plan on their understanding of the above-mentioned. This requirement was not met as eviidenced by: LPA Clayton's interviews and documentation provided by the school which showed that the parent behaved inappropriately in the presence of children in care on at least 2 occasions, which posed a Health and Safety risk to children. 8 Director is to provide signatures/names of each staff members attendance to the department no later than October 27, 2025.

Responsibility for Providing Care and Supervision for Infants (a) In addition to Section 101229, the following shall apply:(1) Each infant shall be constantly supervised and under direct visual observation and supervision by a staff 1 Director wil train the staff on the regulation of Responsibility for Providing Care for infants. Staff will move around the classroom to better supervise and care for infants and during naptime to move every 15 minutes to check on each infant as they sleep. Staff will write what their take away is from the training. person at all times. This requirement was not met as evidence by staff admitting and documentation observed noting staff had been sleep during the time that visual supervision was to be conducted. 8 Director will provide a sign in sheet, Staff will sign an acknowlegement understanding the regulation, Staff will watch the video on Care and Supervising.

Director terminated ( S1) the day of the incident. Director will provide a training to all staff on Personal Rights Staff will watch the video on Cdss website.and write what their take away is. Director will provide Agenda and Sign in Sheets to reflect training. This requirement was not met as evidence by: (S1) disclosed during the interview I grabbed (C1) by the arm and bit him. I left a bite mark on his forearm. I don’t recall which arm. (S2) saw what I did, and she did not condone what I did. This is something that I never done in my life being a teacher. 8

During staff interviews, S1 stated C1 was having a hard time napping and kept moving away from his mat. S1 admitted to picking child up and physically placing him on his mat, which poses a potential health and safety risk to children in care. 8

An unusual incident occurred at the facility on 2/19/2025 and parent reported to the Director on 2/20/2025. The facility reported incident to CCLD on 2/24/2025. This is a potential risk to the health and safety of children in care. 8

An unusual incident occurred at the facility on 9/5/2024 at approximately 10:00am. The Director at that time reported the incident to CCLD on 9/6/2024 at approximately 2:00pm.
