(a) Prior to construction or alterations, the licensee shall notify the Department of the proposed change(s). This requirement was not met as evidence by: Licensee/Director will contact the Department prior to any alterations to existing building and grounds. Licensee/Director will provide a plan in writing to ensure that children are supervised at all times while repairs or work is conducted on site. LPA Richmond observed construction to the outdoor playground. LPA observed two adult males on ladders using power tools to hang metal corrigated panels on the outdoor playground. LPA observed metal corrigated panels on the ground in the outdoor playground area. 8

101227(a) In child care centers providing meals to children, the following shall apply:(1) All food shall be safe and of the quality and in the quantity necessary to meet the needs of the children. Each meal shall include, at a minimum, the amount of food components as specified by Title 7..... 1 The Director will review Title 22 section 101227 regarding Food Services and watch a video with staff, write a statement of undersstanding and ensure the facility has the minium required food components available to children in care. Director submit statment to LPA by POC date. This requirement was not met as evidence by: Based on observations and interview statements, the facility did not provide the minimum required healthy snack options This poses a potential health and safety risk to children in care. 8

101215.1(d) (d)The child care center director, or the substitute director as specified in (f) below, shall be on the premises during the hours the center is in operation. This requirement was not met as evidence by: Based on LPA's observations and interview statements, during the course of this investiagtion; Director has not been present 1 The Director will submit a written statement of her acknowleding the Director Requirements and submit to LPA by POC date at arrival time during LPA's inspection. This poses a potential health and safety risk to children in care. 8

1596.841 Current roster of children provided care in facility required Each child day care facility shall maintain a current roster of children who are provided care in the facility... This standard was not met as evidence by: per licensees statement 1 A complete roster shall be prepared no later than 9/22/2021 and emailed or mailed to the department. there was no roster availble for review. this poses an possible health and safety risk to children in care.
