Based on observation and record review, staff did not have their immunization records in the file during the inspection, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Lead water testing samples taken on 07/05/2022 revealed that levels of lead 1 Director Leslie Diaz demonstrated that all required actions were taken to correct the violation once discovered. As mentioned earlier Director submitted all required licensing forms, supportive documents and plan of correction. Director stated she will submit waiver to waive testing and sampling requirements no later than 10/14/2022. were beyond the allowable amount deemed by the State of California. This is a type B violation which poses a potential Health, Safety or Personal Rights risk to persons in care.
