Based on statements provided by adult interviews revealed the facility operates out of ratio daily, when staff are on break, using restroom, off work, and when assisting children with toileting needs. This poses/posed a potential health, safety and/or personal rights risk(s) to children in care. 8 If a staff member has to go to the restroom, to remain in ratio, a teacher from infant side of yard or admin. will come to preschool side. Based on statements provided by adult interviews and LPAs observations revealed limited availabilty of children's toys, specfically in the outdoor play yard. Additionally, LPAs observed toys and equipment that were rusted, in disrepair, and contained sharp edges. This poses/posed a potential health, safety and/or personal rights risk(s) to children in care. 8 D1 will continue their plan of the opener does a playground walkthough and the closer does a playground walkthrough.

On 11/7/25 and 1/7/2026 LPAs observed preschool children (2 year old and up) 1 D1 stated she can't close the gate that separates the two preschool playgrounds in order to stay in ratio. Facility isn't staffed to have a separate 2 year old playground. playing on the play structure which had a warning label rating the structure for children ages 3-12 years old. This poses/posed a potential health, safety and/or personal rights risk(s) to children in care. 8 However, D1 stated she can post a sign on the play structure advising teachers of age limit and it will by monitored by staff. D1 will email a picture of it posted to jennifer.patel@dss.ca.gov by 1/9/26.

Fire Clearance §101171(a)
All child care centers shall secure and maintain a fire clearance approved by the city or county fire department, the district providing fire protection services, or the State Fire Marshal.This was not met as evidence by: 1 D2 stated that the City of Napa Fire Department will be conducting an inspection on 05/21/24 for an increase in capacity for the preschool license. D2 she is submitting a LIC200A immediately for the increase in capacity. D2 will submit a written plan on how they will stay in compliance with capacity requirements to LPA M. Mohr at Melinda.mohr@dss.ca.gov. Based on interviews with D1 and D2 there were 85 children present at one time. The facility has clearance for 78 children. This poses an immediate health and safety risk to children in care. 8
