Staff will review CCLD training vidoes regarding staff-ratio. Director stated the facility is in the interviewing process. Based on observation LPA observed 15 preschool children and 1 teacher. Which poses an immediate health, safety and personal rights risk to children in care. 8 Goal is to hire staff immediately. Owner or Director will contact assigned LPA when staff has been hired.
Preschool Program with Toddler Component(2) The toddler program shall be conducted in areas, physically separate from those by older or younger children. Requirement has not been met as evidence by 1 Facility owner states she is in the process of hiring addtional staff. Owner will have current teacher to fill in as a substitute and provide a signed declaration. Director will ensure that preschool and toddler Based on LPA 's observation of director caring for preschool and toddlers in the same class in the same room, poses an immediate health, safety and personal rights risk to children in care. 8 will not commingle. Director will ensure that toddler will remain with toddler staff.

1596.954 Carbon monoxide detectors required; inspection. Every licensed child day care center shall have one...carbon monoxide detector...presence of during inspections.Requirement has not been met as evidence by 1 Owner stated she will purchase carbon monoxide and install by 10/24/2025. Owner will contact the department and provide proof of purchase and installment. Based LPA's observation there were no operating carbon monoxide detectors in the facility, Which poses an immediate health, safety and personal rights risk to children in care. 8

Per Director, the facility will go over with staff that parents are allowed access at any point and send LPA Morales a written statenment by POC date. Based on text messages obtained and interview conducted. This poses a potential health, saftey, or personal rights risk to the children in care. 8
