Based on observation, during the tour of the facility, LPA observed the licensee did not comply with the section cited above where 2 bottles of bleach, 1 box of ant killer and 1 can of raid was stored in the sink cabinets which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on interviews, the licensee did not ensure care and supervision of children in care since after-school staff did not realize a child had been left unattended in the facility transportation van for approximately 45 minutes, which posed an immediate risk to the health and safety of children in care. 8 In addition, acting Director will submit a plan of checking in after-school children to ensure this does not happen again and will submit to LPA via email. Director will provide LIC 9224 to all parents enrolled for a period of one year and keep a copy in the child's file for review by the Department.
