During the tour of the infant classroom, LPA observed an infant sleeping with a pacifier in their mouth in a rocker with the "vibration" on. The child was strapped in by the belt the rocker comes with. Staff stated the infant had just fallen asleep, but LPA was in there for at least 3-5 minutes where the infant was sleeping in the rocker before they were picked up and placed in a crib. This poses an immediate health, safety or personal rights risk to persons in care.
Based on observation during the facility tour, LPA found that the licensee did not comply with the section cited above. LPA observed a "Fisher Price walk bounce and ride pony" walker in the infant classroom. This poses an immediate health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above in 4 out of 4 persons. The facility was under the impression that 15 minute checks were only required up until the age of 12 months. There were no 15 minute checks found in those files of children between the ages of 12-24 months. This poses/posed a potential health, safety or personal rights risk to persons in care.
Facility agrees to keep an infants needs and services plan on record and made available to the department to review. Assistant director/director agrees to have a meeting with infant staff and ensure they understand the importance of needs and services plans for infants in care. During a previous inspection, an infants needs and services plan was not kept in the child's file and was not available to review by the department.
