101238 (a)The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This was met not met by: 1 The Director stated that the owner has been made aware of the condition and will repair the wall in Room #7. The Director stated that the wall is expected to be repaired by 9/25/26. The Director will provide proof of correction by submitting photographs of the repaired wall to the SDRO. Based on the Analyst’s observations and the admissions of the Director and one (1) staff member that the wall had not been repaired, the facility was not in compliance with the cited regulation. The condition poses a potential risk to the health, safety, and personal rights of the children in care. 8

Based on LPA's observations, records review, and interviews, the Facility did not comply with the section cited above as there were rat feces found in the facility. This poses a potential health, safety or personal rights risk to persons in care. 8 Sanitation regulations with the Administrator. Administrator states facility will send the San Diego Regional Office a written plan of correction on the new cleaning protocol plan and proof of continued pest control services by 12/9/22.

Based on record reviews and conducted interviews, Licensee did not ensure children in care were supervised, which poses as an immediate risk to the health and safety of children in care. The director stated a written plan of correction describing new procedures on supervision and steps on releasing children from the facility and training agenda including staff attendance sheets will be submitted to the Department by 06/17/2022.
